He completed a fellowship in sports Medicine and Arthroscopy at The Hospital for Joint Diseases in New York City in 1993.
Dr. Bazos served as Medical Director of Yankee Stadium for seven years, and continues in this capacity at Madison Square Garden. He is certified by The American Board of Orthopedic Surgery. In addition to his practice in the New Milford and Danbury area, Dr. Bazos is a member of the Sports Medicine Department at The Hospital for Joint Disease in New York City, where he maintains operating privileges and keeps office hours.

David is a passionate and dynamic physical therapist who truly embodies and believes in the art of movement. He believes in a holistic approach to treatment addressing the body as a whole unit in attempting to search for the cause of dysfunction rather than the symptom. He does not follow the classic "cookie cutter" approach to treatment but rather uses cutting edge assessment and treatment techniques to address each individuals' goals. David partnered with Living Well Balanced because of the shared identity when it comes to addressing patients goals.
Growing up, College and DPT school-
David hails from the beautiful city of Montreal, Quebec where he resided until graduating high school. He then attended Yeshiva University where he majored in Biology. During his time at Yeshiva University, David gradually became fascinated by the human body while becoming a TA in anatomy and decided to combine his love for helping people, anatomy and movement to pursue a career in physical therapy. During his time at Yeshiva, David also became a certified personal trainer and trained athletes and other students.
After Yeshiva, David attended Columbia University, College of Physicians and Surgeons where he obtained his DPT- Doctor of Physical Therapy where he pursued the orthopedic tract in his final year. While at Columbia, David interned at several prestigious orthopedic and sports medicine clinics such as The Nicholas Institute of Sports Medicine and Athletic Trauma at Lenox Hill hospital and SPEAR Physical Therapy.
David started his career at Metro Physical and Aquatic Therapy here he quickly rose to the title of Senior Physical Therapist. David treated a diverse patient population ranging from orthopedic and sports medicine patients, athletes, weekend warriors, neurological patients, geriatrics and post-operative cases. David then moved to Connecticut and started working at Orthopedic and Neurosurgery Specialists treating high level athletes, sports injuries and many complex post-operative cases. David recently left to join and partner with Living Well Balanced to provide a comprehensive multi-disciplinary approach to treatment.
Dr. Howard Anthony Rose is Board Certified by the American Board of Orthopedics and performs the latest arthroscopic procedures for rotator cuff repairs, shoulder stabilizations, anterior cruciate ligament reconstructions, and meniscal repairs. He also performs joint replacements of the hip and knee using minimally invasive techniques. He is currently an Assistant Attending Orthopedic Surgeon at Hospital for Special Surgery and an Assistant Clinical Professor of Orthopedic Surgery at Weill Cornell Medical College.
Dr. Rose received his undergraduate BA degree cum laude from Harvard College, where he played ice hockey. He completed his medical degree from George Washington University School of Medicine. He completed a surgical internship at Tufts-New England Medical Center in Boston and his orthopedic residency at Hospital for Special Surgery. He is fellowship trained in both Sports Medicine and Adult Reconstruction (joint replacement) from Harvard’s Brigham and Women’s Hospital in Boston and is a former assistant Team Physician for Harvard University Athletics, Long Island Rough Riders professional soccer club, and Brooklyn College.
Dr. Rose is an active member of many organizations and has authored numerous peer review articles. He has an additional office in Long Island.

A specialist in orthopaedic sports medicine, Aravind Athiviraham, MD, cares for patients with athletic and overuse injuries, including anterior or posterior cruciate ligament tears, meniscus or cartilage injury, patellar or shoulder instability, and elbow ulnar collateral ligament tears. He is skilled in minimally invasive and arthroscopic procedures of the knee, shoulder, and elbow. In addition, he has received advanced training in complex procedures of the knee and shoulder, including revision knee ligament reconstruction, cartilage restoration, meniscal transplantation, open treatment for recurrent shoulder instability, and robotic-assisted knee reconstructive surgery.
Dr. Athiviraham is dedicated to the care of his patients and stays up-to-date on the latest techniques in sports medicine. He enjoys getting to know his patients and working with them to choose the best treatment option, by fostering a combined patient-physician decision making approach.
Dr. Athiviraham's current research interests include establishing a cadaveric biomechanical study for superior capsular reconstruction using human dermal allograft to optimize patient recovery from surgery and allow early return to sports activity.

Dominika Piestrak originates from Krakow, Poland. She graduated with a Master's in Physical Therapy from Jagiellonian University in Poland. Dominika has lived in multiple cities and worked in numerous areas of expertise in her pursuit of a broad range of training. Her professional career started in Paris in an orthopedic private practice where she was trained in the treatment of Scoliosis (TGS 3D). Dominika then pursued advanced clinical education at Kaiser Permanente hospital in California. She completed a Proprioceptive Neuromuscular Reeducation residency program which focused on patient care post stroke, brain injury, and other neurological conditions. She is also a Certified Running Coach through RRCA and Certified FRC Mobility Specialist (FRCms).
Dominika's treatment philosophy is guided by an exploration of how a patient's unique body structure/composition is connected to their presenting condition. She offers 60 minute one on one patient care and combines manual therapy (Maitland, Cyriax and the Mulligan approach) and neuromuscular reeducation with specific therapeutic excercises to help her patients acheive optimal results. Dr. Gladstone is a board certified orthopaedic surgeon. Dr. Gladstone is a specialist in sports medicine, arthroscopic surgery, and knee and shoulder reconstructive surgery. He attended Dartmouth College and Tufts University School of Medicine. He performed his orthopaedic residency at Columbia-Presbyterian Medical Center's New York Orthopaedic Hospital.
Following his residency he sub-specialized with a fellowship in sports medicine at the American Sports Medicine Institute in Birmingham, Alabama under the tutelage of Dr. James Andrews and Dr. William Clancy. In 1996, he joined the Department of Orthopaedics at Mount Sinai as an assistant professor and specialist in sports medicine.
Dr. Gladstone's areas of expertise include arthroscopic surgery of the shoulder, elbow, knee and ankle, ligament reconstruction of the knee, shoulder stabilization procedures for instability and dislocation, rotator cuff problems from bursitis to rotator cuff tears and knee and shoulder total joint replacement. As a result of intense exposure during fellowship, Dr. Gladstone is highly skilled and up-to-date in the latest techniques of arthroscopic surgery. This type of surgery, which uses fiberoptic pencil-thin camera, can reduce skin and muscle incisions and facilitate rehabilitation and a faster return to activities.
